added button to /workspaces page for creating a std imaging capability request

I have a couple aesthetic qualms; namely, the color of the button being the same as calibration, the button having an icon while all the others don't, and the capitalization of the button text not matching the other buttons.

Our temporary sandbox that will get deleted soon is very important! and we must make sure it is as beautiful as possible.

I like this, but it's not clear that you still need to supply an email address. Ideas on how to do that?

Yeah, good call. The easiest way would be to make the email address input required. Another way would be to toggle the Imaging input fields based on a radio button or something like that. Wdyt?

Another idea would be to split the form into three areas: the null area, which needs no input fields; the download and calibration area which needs the three fields we know and love; and the imaging area, which needs the two fields (three?) plus email.

Thoughts?

I agree that the sandbox is important, at least for now. We could also make separate forms altogether. You could use tabs to choose which form you're filling out, or you could have a drop-down for choosing the capability, which changes which controls you see.

I realize now that three areas is about the same thing as what I said.

I think Nathan's version - without the tabs - would be easier to implement this week and would help make the required inputs more clear

Fresh commit and new screenshot! Can you all take another look?

My only remark is that it would probably look a little less jumbly if the buttons were left- or right-aligned rather than centered.
